Age Requirement. You must be 18 years of age or older to use this service. By using our service, you represent and warrant that you are at least 18 years old and have the legal capacity to enter into these Terms of Service.
MyHospitalHelper's primary service is providing your Patient Planner—a personalized PDF that helps you organize your questions and story for when doctors and the medical team visit (in the hospital, nursing home, or emergency room). You complete a short process (typically about 8 minutes); we deliver your Patient Planner by email and/or by PDF download. No login or account is required. We never sell your information, and you stay in control of your planner.
Optional 45-Minute Concierge Call. You may choose to pay separately for a 45-minute video or phone call with one of our doctors for personalized preparation help. This call is optional, educational and administrative only, and is not medical advice. It is available for purchase separately from the Patient Planner.
Your completed Patient Planner is delivered by email and/or by PDF download immediately upon completion (typically within minutes).
Educational Tool Only. MyHospitalHelper is a preparation tool that helps you organize your questions and story for medical team visits. We do not provide medical care, treatment, diagnosis, or medical advice. We do not provide legal advice. We are not a healthcare provider, health plan, or law firm.
No Doctor-Patient Relationship. Use of our website or participation in an optional concierge call does not create a doctor-patient relationship. Our service is not a substitute for clinical care. If you have a medical emergency, call 911 or your local emergency services immediately.
All medical decisions should be made with your treating healthcare providers. Our role is to help you prepare so you can get more clarity when the medical team visits.
Scope of Refund Policy. The M.D. Guarantee and refund policy described below apply only to the Patient Planner. All sales are final for the Concierge Call. No refunds will be issued for Concierge Call purchases.
M.D. Guarantee. We stand behind our Patient Planner. If you show your Patient Planner (MyHospitalHelper) to your doctor and it does not help you get clarity, you may request a full refund. No questions asked. Email us at Contact@HealthCareProxyMD.com within 30 days of receiving your Patient Planner. Please include your name, the date you received it, and a brief explanation.
What "Delivery" Means. "Delivery" means the date we deliver your Patient Planner to you by email and/or by PDF download. If by email, this is the date shown in our email to you; if by PDF download, it is the date you complete the process and receive or download your PDF.
Refund Process. Refunds are issued back to your original payment method through Stripe. Processing typically takes 5–10 business days after we approve your request. We reserve the right to deny refund requests submitted after the 30-day window or for any request we determine, in our sole discretion, to be fraudulent or abusive.
Refund Limitations. One refund per purchase. Refunds are not available after the 30-day window. We do not offer refunds for chargebacks initiated directly with your bank or credit card company. If you initiate a chargeback, we may dispute it and may not process a separate refund request.
Payment Required to Schedule. If you purchase the optional 45-minute concierge call, payment is required to schedule. We accept payments through Stripe. After payment is processed, we will email you within 48 business hours to schedule your video or phone call.
How to Reschedule. If you need to reschedule your call, contact us at Contact@HealthCareProxyMD.com as soon as possible. We will work with you to find a new time.
Cancellation Policy. If you cancel more than 48 hours in advance, you may reschedule without penalty. Late cancellations (less than 48 hours before your scheduled call) or no-shows may result in forfeiture of your session, though we will make reasonable efforts to accommodate rescheduling when possible.

MyHospitalHelper provides the Patient Planner. How you use it with your doctors and care team is up to you. We do not control how your doctor or facility uses or documents the information you share.

To the maximum extent permitted by law, MyHospitalHelper and its physicians, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, whether incurred directly or indirectly, or any loss of data, use, goodwill, or other intangible losses resulting from your use of our service.
Our total liability for any claims related to our service shall not exceed the amount you paid for the service.
We may update these Terms of Service from time to time. We will notify you of any material changes by posting the new Terms on this page and updating the "Last Updated" date. Your continued use of our service after changes become effective constitutes acceptance of the updated terms.
